Apostilles attached to Certificates of Loss of Nationality (Form FS-348) can be obtained at:

The request letter must clearly state what is being requested, be notarized and accompanied by a clear copy of a picture ID, such as a driver’s license or other government issued identification card. Also, it should identify the person’s full name, date and place of birth, justification for the record, and include contact and return address information. Enclosing a copy of the Certificate of Loss of Nationality may expedite the search.

The fee for all vital records is $50.00.
