Apostilles on Consular Reports of Birth Abroad or Consular Reports of Death Abroad

Apostilles attached to Consular Reports of Birth Abroad (FS-240) or to Certifications of Birth Abroad of a U.S. Citizen (DS-1350) can be obtained at:

The request letter must be notarized and accompanied by a clear copy of a picture ID, such as driver's license or other government issued identification card.   Also, it should clearly identify the child(ren)'s full name, date and place of birth, justification for the record, and include contact  and return address information.  
The fee is $30.00 for the first document and $20.00 for each additional document (per subject).

If a certification of “all” passport records (copies of records that include passport records and FS-240 with attachments) is requested, a similar written request should be submitted to:

The fee is $30.00 for the first document and $20.00 for each additional document (per subject).
