NonImmigrant Visa Forms

Please click on the links below to download and fill out the nonimmigrant visa forms. The only acceptable format for the DS-156 is the Electronic Visa Application Form (EVAF) format.
All forms, EVAF or other, must be filled out in English.  A Dutch PDF version of the DS-156. the DS-157 and the DS-158 is available to help you fill out the English version.

-Nonimmigrant Electronic Visa Application Form DS-156—EVAF (ALL APPLICANTS MUST COMPLETE THIS ONLINE, THEN PRINT AND SIGN THE COMPLETED FORM.)
Please complete the forms and press "continue" at the bottom before printing. Make sure the third page, containing the 'scrambled' barcode, has printed correctly.  See the sample barcodes on the right.

-Supplemental Nonimmigrant Visa Application Form DS-157 (for all male applicants between the ages of 16 and 45, inclusive)

Click→ DS-157 (not an EVAF, thus no barcode will print)

-Supplemental Nonimmigrant Visa Application Form DS-158 (for all applicants seeking a student or exchange visitor visa: J, F or M)
Click→ DS-158 (not an EVAF, thus no bar code will print)
 

-Investor's Visa  Supplemental Form DS-156E (required for all E Treaty Trader/Investor visa applicants)
Click→ DS-156E (Not an EVAF, thus no barcode will print)

- The Importance of the Barcode -

The third page of your DS-156 application form has the barcode.  This barcode will be scanned at the Consulate General.  If the barcode does not print correctly the first time, it is necessary that you reprint your DS-156 form.  You can check your barcode by comparing it to the following samples:
