Changing your name in your passport
If you have changed your name by any other means than marriage or divorce, you must apply in person for a new passport to be issued in your new name. If you have changed your name through marriage or divorce, please check our Name change by mail page.
Please note: You will need to submit documentary evidence such as a court order, royal decree, or other satisfactory evidence to support a change of name. Only originals or certified copies will be accepted. Documents presented must be in English.
- Download, print and complete, but DO NOT SIGN, the application form DS-11.
- Bring your current passport.
- Provide one U.S. format, 2" x 2" (5 Cm. x 5 Cm.) photo. See our photo specifications page.
- Bring the original documentation supporting your name change. This can be a variety of instruments, e.g. a royal decree, a court order, or a deed poll. Or it can be documentation showing that you can claim the new name by customary usage over a long period of time - generally at least five years. - Set up an appointment online to visit the Consulate. On the appointment website choose the option "Apply for a U.S. passport for the first time.", to get the right type of appointment. Please read our entry instructions.
Click here to go to the appointment website. - The appropriate Passport Services fee must be paid in cash, euro or dollars. PIN payments or checks cannot be processed.
Processing time
Passports are printed in the United States with a secure photo-digitized image of the bearer. You should allow for up to 10 to 15 working days for your application to be processed.
