You must submit the following to replace an emergency limited validity passport:

1. The DS-5504 application form – see our Tips for completing the DS- 5504 form;

2. Enclose your emergency passport;

3. Two photos of the 2" x 2" (5 x 5 cm.) format. See our photo specifications page.;

4. Enclose a self-addressed envelope;

5. Mail your passport and application to:

6. There is no fee for this service.

Please note the following:

If you received your limited validity emergency passport more than one year ago, you will need to complete and submit the DS-82 application form – please see our section on Adult Passports.  However, for minors, the DS-11 application form must be completed and submitted in person.  Please see our section on Passports for Minors for further details.  In both cases you will have to pay the full fee for the new passport.
